Bad breath is a considerable social problem which can cause significant psychological distress.
1 in every 4 people suffer from bad odour from mouth.
In medical terms bad breath is known as Halitosis.
People spent on deodorant type mouth oral rinses, mints to manage bad breath.It would be preferable to spend this money on a proper diagnosis and care instead of short term masking attempts.

Why does it occur?


1.Bad breath mainly occurs as a result of improper oral hygiene.

2.Deep cavities with food lodgement are harbour of various kinds of numerous bacteria which cause breakdown of food particles and release sulphur and amines in mouth.

3.Dry mouth with less saliva causes increase bacterial load in mouth.

4.Accumulation of  food coating on tongue.

5. Ulcers in mouth.

6.Bad breath is also a result of other systemic conditions of body such as bronchitis, liver and kidney diseases, sinusitis and hormonal imbalances.

Treatment

1.Cleaning of tongue which can be carried out with normal toothbrush or tongue scraper.

2.Maintaining proper oral hygiene by brushing twice daily.

3. Chlorhexidine mouth washes and gargles.

4.Regular dental check ups and cleaning of teeth if required.

5.Treatment of underlying systemic diseases.

EARLY CHILDHOOD CARIES: AN ALARMING SITUATION


Early childhood caries are cavities that form in your child's tooth before he or she is 6 years old.

             The cavities can start as soon as the tooth starts to erupt!!!!

Early Childhood Caries are also called BABY BOTTLE SYNDROME OR NURSING CARIES. Tooth decay formation starts when sticky or sugary foods remain in contact with child's tooth for a longer period of time. This includes formula, breast milk, fruit juice, cow milk or any sugary drink given in a bottle,or breastmilk during breastfeeding. Your child may have less saliva and swallow less often while he or she  sleeps.This means liquid stays on his or her teeth longer than when he or she is awake.

Mothers with untreated dental disease present a very high risk of early childhood caries to their children due to transmission of bacteria through breastfeed.

The ECC can significantly affect the child's quality of life as it may lead to infection, swelling, pain and other symptoms. The ECC affcts children after eruption of milk teeth until age 6.

In primary teeth,dental caries is a reversible and preventable disease if treated in early stages,but when left untreated it leads to pain, infection, alteration in growth and develpment, premature tooth loss,speech disorder,increase in treatment costs,loss of confidence and also affect successor permanent teeth.

The habit of feeding young children at night using nursing bottle or sippy cups will increase risk of cavities.This habit will elevate the chance of dental caries due to low flow rate of saliva at night.

HOW TO DIAGNOSE ECC

The earliest signs are white spots along the gum line,near the upper front teeth. You may not be able to see these spots. Your child may not have any symptoms if the dental cavities have just started to form. When the dental caries reach deeper parts of your child's tooth, he or she may have pain. The pain may get worse when your child chews or eats hot and cold foods.

   
       PREVENTION AT HOME
  1. Lift the lip and check monthly.
  2. Healthy foods and drinks.
  3. No sippy cups.
  4. Use of flouride toothpaste.
  5. Dental appointment before their first birthday.
  6. Wipe infants gums and tongue.
      TREATMENT OF ECC


1. Flouride treatments may be given at dental office or at home.                                  
2. Dental fillings are also a treatment of choice in early cases. 
3. Regular visit to dentist may prevent formation of cavities.
                               

4. Avoid giving milk bottle to your child during night time. If you do need to feed your       baby,wipe his or her teeth with a clean washcloth after feeding.This will remove           milk or formula before you lay him or her to sleep.
5. Teach your child to use regular cup for drinking as early as by 12 months.
6 .Foods which are low in sugar should be preffered over candy,cookies and soda.

7. Sweetened paccifiers should be stopped.

The first  bristle toothbrush appeared about the year 1600 in China, was first patented in America in 1857.                                                                
The daily use of a toothbrush and other oral hygiene aids is the most dependable way of achieving oral health benefits to everybody. Brushing twice daily is the secret to healthy teeth and gums as well as bad odour!

Use of hard toothbrushes, vigorous horizontal brushing and use of abrasive toothpaste may lead to abraded teeth and bleeding gums.Softer bristles are more flexible. Soft bristle toothbrush is best for kids and parents.It allows you to only brush hard enough to clean the plaque off the teeth.Firm or even medium bristles or vigorous scrubbing in horizontal motion may cause damage to gums and enamel.
And teach your kids the 2+2=4 rule ie brush two times per day for two mins.

                                                             
                                                                     RECOMMENDATIONS:

1.Vigorous brushing is not necessary.
2.Soft nylon bristle toothbrushes clean effectively ( when used properly) remain effective for a reasonable time tend not to traumatize gums.
3.Toothbrushes need to be replaced every3 months.
4.Powered toothbrushes permit slightly better tooth brushing
                                                               
  
                                                            TOOTH BRUSHING TECHNIQUES

1. Place the bristles towards gums and swipe out the plaque in outward motion.
2. Exert gentle pressure using back and forth motion.
3. Place the bristles on teeth and round motion is recommended in outer and inner areas.
4. Complete several strokes in same position starting from posterior teeth and then anterior teeth.
5. Complete brushing on teeth of both upper and lower jaws.

  1. Begin early
    Toddlers who are accostomed to having their teeth cleaned tend to be more responsive to learning how to brush their teeth themselves.Parents should clean their childs mouth from infancy. Beginning with wiping baby's gums with a clean damp cloth after feedings.
  2. Brush your teeth together
    Make teeth brushing a family activity. Young children love to imitate their parents. Model how to practice good oral hygiene by brushing and flossing in front of your child and let them try themselves.
  3. Let your child pick a toothbrush
    Allowing a child to choose his/her own toothbrush helps makes the process of learning a fun!
  4. Get them flavoured toothpaste
    Children are very well aware of cartoon characters and so by their names toothpastes are also available. They attract the children to brush their teeth.
  5. Play music
    Playing rhymes while brushing makes the learning more enjoyable!

Tooth Decay ( Dental caries) is destruction of tooth structure due to constant deposition of plaque  (sticky film of bacteria) which when comes in contact with sugars releases acid and form cavities.

Bacteria in mouth ferment sugars to form cavities which eventually destoys enamel, dThis can happen if you eat or drink often, especially foods and drinks containing sugar and starches. The repeated cycles of these “acid attacks” will cause the enamel to continue to lose minerals. Over time, the enamel is weakened and then destroyed, forming a cavity.entin and innermost layer pulp. This is an irreversible process. It is more common among children.

Young children are at risk for “early childhood caries,” sometimes called baby bottle tooth decay, which is severe tooth decay in baby teeth.

Our teeth comprises of 3 layers:

Outermost hard and mineralized tissue known as ENAMEL, inner layer DENTIN and soft tissue which is innermost known as PULP.

ENAMEL is hardest tissue in the body equivalent to bone. It is composed of hydroxyappatite crystals.DENTIN is a calcified tissue covered by enamel in crown part and cementum in root part which surrounds the pulp. It consists of hydroxyappatite organic material and water. It also consists of microscopic channels called dentinal tubules which has degree of permeability.PULP is a connective tissue that consists of nerves, blood vessels and other cellular components.

                                   SYMPTOMS OF TOOTH DECAY

1.Toothache: It can be continuous sharp and acute or it may be dull aching.
2.Tooth sensitivity: tenderness on having hot cold and sweet.3.Brown back spots appearing on your teeth.4.Bad breath5.Food lodgement around gums and teeth.

                                            PREVENTION OF TOOTH DECAY

1. Brushing your teeth twice daily, especially before going to bed with a flouride containing toothpaste prevents cavities in mouth.

2.Include smart diet in your lifestyle which include balanced and nutritious diet. Avoid intake of sugary food, junk snacks, sugary drinks, candies and chips at daily basis.

3. Rinse your mouth with mouth wash daily as its antiseptic action kills large number of bacteria.

4. Ask your dentist to guide you about dental sealants applied on teeth for prevention of decay.

5. Visit dentist regularly after 6 months for check ups.


                                                                TREATMENT

If tooth decay is limited to enamel and dentin then it is reversible and treated through restorative procedures like fillings. If it reaches the pulp then irreversible destruction to the tooth occurs which can be treated by otherdefinitive and restorative procedures like RCT, apicectomy,etc.

Scaling in layman terms is called cleaning of teeth is a procedure of oral hygiene prophylaxis to remove the plaque and tarter which gets deposited around the teeth and gums over time and eventually causes inflammation of gums and leads to early stage of gingivitis.

PLAQUE is a biofilm of millions of bacteria which is sticky, colourless and soft in consistency.
This layer can easily be removed with brushing. While on the other hand DENTAL CALCULUS is yellowish brown hardened mass of plaque which gets mineralised over time and can only be removed by ULTRASONIC SCALERS by the process called SCALING.
Improper brushing and poor oral hygiene lead to heavy deposits of plaque and calculus.
Calculus is a harbour of numerous bacteria and other microorganisms which if not removed regularly lead to various complications like bleeding from gums, pockets, infection in bone and mobility of teeth over time.
Scaling is performed by dentist every 6 months to 1 year to prevent the gums and teeth from potential complications.

MYTHS ABOUT SCALING

1. MOBILITY OF TEETH

Scaling doesnot cause mobility of teeth. It is the chronic deposition of plaque and calculus around teeth and gums which leads to progression of gum disease, bleeding of gums and weakening of bone.
Infection in bones causes loss of support and teeth become mobile.
Scaling on the contrary stops the mobility of teeth!

2.CAUSES GAPS BETWEEN THE TEETH

Again the deposits around teeth cause recession of gums (gums change their position) and create spaces between them. After scaling is completed tarter gets removed and it gives a feeling of gaps in between teeth.

3. SCALING IS PAINFUL

It is actually a painless procedure. Sensation of slight scraping of hardened mass of deposits is felt during the procedure.

4. IT CAUSES SENSITIVITY

After the scaling is completed our teeth and roots gets exposed to the environment which may cause transient sensitivity in some patients but it usually vanishes within a day or two.

5. ENAMEL IS REMOVED

Enamel is the outermost and hardest layer of tooth. It doesnot  easily gets removed by scaling.
Scaling removes only the calculus deposits around teeth.
After scaling is completed polishing is done to create smooth surfaces on tooth.

REALITIES OF SCALING

1. One should visit the dentist every six months to get the scaling done.

2.Dentist on oral health examination would advise you how to maintain oral hygiene, brushing techniques and at what interval scaling is required.

3. Scaling prevents from gum disease and periodontal pocket formation.

Neglecting your teeth could be seriously bad for your health!!

Its no secret that a lackadisical approach to dental care leads to fillings and gum diseases but latest evidence suggests that it could also cause DIABETES AND HEART DISEASES!!!

PERIODONTAL DISEASES are infections of structures around teeth which include gums,periodontal ligament and bone. In early stage of periodontal disease, Gingivitis( infection of gums) affects only gums and later on it may relate to Diabetes and Heart disease.
Gum bleeding is a sign that you are risk of gum disease.

Continued gum bleeding may be due to serious medical conditions like  Leukaemia and Platelet disorders.

Uncontrolled diabetes are more likely to develop Periodontal infections because they are more susceptible to contracting infections due to compromised immune system.
Opportunistic mirobes and bacteria around gums can elicit an inflammatory response.

HOW GUM INFECTION OCCURS

It starts with a white sticky layer  on the surface of teeth and around the gums which is a niche of various tupes of bacteria, starch and sugars along with saliva.
Consequently this plaque mineralises to form Calculus which is yellow hard coating and cannot be removed with toothbrush.

HOW DIABETES IS LINKED TO PERIODONTAL DISEASE

1. High glucose levels in saliva promote growth of bacteria leading to gum disease.
2 .People with diabetes who smoke are far more likely to develop gum disease than people who smoke and do not have diabetes.
3.Resistance to infection in diabetes is slowed down as compared to non diabetic person.
4. Poor oral hygiene aggravates the situation.
